Disk periodically loses interface connectivity

Gurus,

I have a question that I hope you can answer. I have a StorEdge 5200
configured in a dual-loop config. One disk on the back periodically loses
all interface connectivity to A1 and B1 GBIC. Other disks stay up. I've
swapped the disk only to have it stay active and then drop connectivity
again. I did a luxadm -v display BOX and see this result:

Any ideas on why this keeps losing connectivity? I've read the literature
but perhaps my searches aren't worded properly.
