Why system is brought down to maintenance mode automatically?

I lost connection to one of our systems in network, could not ping nor
telnet to it. I have to access it via terminal server and found it was
at maintenance mode, it required me to run "fsck -F ufs
/dev/rdsk/c0t0d0s0" to repair.

My question is what brings system down to maintenance mode like this?
And what causes root partition (/dev/rdsk/c0t0d0s0) to be corrupted?
Please help.
