..synopsis..
..your prtdiag shows you have three cpu/memory boards in slots 0, 2, and 4,
4Gbs of memory, 2Gbs on board 0 and 2Gbs on board 4, and one i/o card
operational..it also shows that the memory on board 2 has failed because of
unavailable ac..i presume you have a second cpu on that board as well,
though i see no error indicating such..multiple failures with fans and
power..

 ..fix action..
..cpu..i presume you have two cpu's per board..board 2 only shows one
online..it's possible the second is not showing up because of the ac
unavailable error, or the cpu in slot 1 on board 2 is bad, or needs to be
re-torqued..the board could also be bad..a quick check is to swap it with
another board and see if the problem follows the board or stays..

..memory..i presume you actually have a total of 6Gbs of memory although
your configuration shows a total of only 4Gbs (2Gbs on board 0 and 2Gbs on
board 4) with board 2 not registering because of the ac unavailable
error..that error is probably caused by a defective power supply..

..i/o..looks okay..

..failed fru..probably bad power module for that slot..a quick check is to
swap it with another module and see if the problem follows or stays..

..detected system faults..there were multiple failures in this area..AC Box
Fan failure, Key Switch Fan failure, AC Power failure, System 5.0 Volt
Precharge failure, System 3.3 Volt Precharge failure, Peripheral 12 Volt
Precharge failure, Auxiliary 5.0 Volt Power failure, Peripheral 12 Volt
Power failure, PROM detected failure, and Board 2 fault: PROM detected fault
which leads me to believe your peripheral power supply is bad..
..also, check the input power source to your system..hope this helps..c-ya

13 Now all has been heard;
here is the conclusion of the matter:
Fear God and keep his commandments,
for this is the whole duty of man.
