track disk/file access by process id

Hi,
Can anyone suggest a tool to track disk access by process id?
The machine is doing a lot swapping and we see a lot of page fault.

Also, given a file, how can I find out which process is r/w to it?

We want to identify the process and see if we can put the right amount of load
to the server.
