Sun Quad Ethernet Adapter

I am attempting to install a Sun PCI Quad FastEthernet (qfe) adapter
into a PCI-based E220R machine, however, I cannot get the driver to
attach to the device.

It appears that the firmware is recognizing the 4-port Ethernet adapter
hardware on the PCI bus, but when the system boots it does not recognize
the card as expected.
For example, I do not have a /dev/qfe file and my /etc/path_to_inst file
does not contain anything referring to qfe. Also, here is the output of

I have removed and re-added the SUNWqfed package and it's latest patch
several times to no avail. Unfortunately, I do not have the CD that
normally comes with the card. If there is additional software I need on
this CD, I would appreciate if someone could contact me and possibly
send a copy.
