Config Help on Sun PCI Dual FC Card - X6727

V880, Solaris 8, VxVM 3.2

This V880 has 1 base backplane with 6 disks and was purchased with a Sun
StorEdge PCI dual FC card (QLA2200?/X6727?). I'd like to use port A to dual
path my internal disks and to use the other port (B) to connect fabric-wise
to my tape library. One of you recommended I use QLOGIC's driver QLA2200-3
which I did and got the following proof that the driver is indeed
recognizing the 3 instances - the on-board plus the two ports on the card
itself:

Questions:

My sd.conf is properly setup, I added persistent binding entries in my
qla2200.conf but I cannot see the disks in format or inquire. Would any of
you be kind enough to give me a sample of how your qla2200.conf might look
like and how I configure 1 port for loop and the other port for fabric?
