Thanks to all who responded. Here's what I did per

Tom Myers' guidance:

Umount the volumes in the volume group.

Umount /dev/vg02/lvolx

Deactivate the volume group

# /usr/sbin/vgchange -a n vg02

Wrote the LVM information to the new drive using vgcfgrestore:

# /usr/sbin/vgcfgrestore -n vg02 /dev/rdsk/c0t3d0 (here's the importance of
using vgcfgbackup)

Re-activated the volume group:

# /usr/sbin/vgchange -a y vg02

Subject: [HPADM] replacing bad disk in physical volume

We have a D390 running 10.20 that has a volume group comprised of PV1 and
PV2. The volume group vg02 using PV2 and is striped and
mirrored. c0t3d0 is part of PV2 and died, but the volume group is
still working. What is the correct procedure for removing the bad disk from
physical volume, replacing it with a new disk, and re- establishing the
extents. Thanks in advance. Will summarize.
