Thanks to the few that responded although none of the suggestions solved the problem.
We called HP and finally found someone with the answer. The problem is caused by running Veritas Netbackup on a machine that is running an unpatched version of OnLineJFS. The solution from HP was, what else, install the latest OnLineJFS cumulative patch (PHKL_24066).
