I've seen this same kind of output when I've had dead ssa-disks. Try:

        ssaidentify -l pdisk0 -y

to see if you can turn on the identification ligth. I suspect you're
going to get a 'hardware error'-message when running this command.
Report it to IBM, and get the disk replaced.
