Why not run diagnostics and verify the disk (certify and even format the
disk). If you got problem you got a bad disk and just replace it.

Imho, just try to make rmdev hdisk, then rmdev pdisk (equivalent to hdisk)
then cfgmgr.

M80 running AIX 4.3.3
I was trying to add hdisk11 to a volume group. I am
getting the following error:
0516-024 hdisk11: Unable to open physical volume.
Either PV was not configured or could not be
opened. Run diagnostics.
0516-792 extendvg: Unable to extend volume group.
However, I was able to add another hdisk(hdisk16) in
the same pack to this volume group without any
problem.
I did 'rmdev -dl hdisk11' and ran 'cfgmgr'. I
received the same error when I ran 'extendvg vgname
hdisk11'.
Any ideas, TIA
