dont you have anything in errpt ?
if you reply, include
lscfg -vpl ssa0
lscfg -vpl hdisk2
lscfg -vpl pdisk0
I assume there is some physical problem, not a logical one; based on the
output of ssa_rescheck .
Also check your ucode-level with those stated at the ssa-support site (
somewhere at http://www.storage.ibm.com/ ; my ssa bookmark doesnt work
anymore).
