Defects of a data medium due to technical deficiencies are quite common. Pertained are mass storage devices such as hard disks, tapes, and cartridges. Hard disks can be destroyed by crashes of the read/write head, while tapes and cassettes can be damaged by direct mechanical impacts. Even CD's can be rendered useless by superficial scratches. Diskettes are particularly vulnerable to failure. Often, it turns out that reading from, or writing to, a diskette is no longer possible.
Example:
In a medium-sized company, construction work generated dust particles which found their way to the magnetic disk of the computer used in that firm, causing a head crash and resulting in data being deleted.
